ZenMINDpod will collect, store and use personal information about you, in line with the Data Protection Act 2018.
This information includes the following:
- Personal details.
- A record of meetings and communications between us.
- A record of any actions we take as part of your support.
- Information provided by you from relevant others for the purposes of supporting you, at your request.
In order to support you, I may need to share this information with other people, that we will agree, if you wish, or in order to help you access specific specialist support. Therefore I may disclose information about you to your GP (doctor) or other relevant professionals or organisations if I feel it would be beneficial to you, and in certain circumstances to your nominated emergency contact if this has been recorded during your initial assessement and discussions.
Sharing will be on a strictly on a ‘need-to-know’ basis in order to provide support or meet legal requirements. Wherever possible ZenMINDpod respects your wishes regarding the use, storage and sharing of information. However, I may not be able to do so in certain situations such as disclosure of potential harm to self or others, or of criminal activity. I will store this information securely for up to six years after your sessions. Which will later be destroyed, deleted.
